1. All participants must complete a Registration Form
   (Please print clearly - all information is required).
   

2. Every participant, or authorized guardian, must sign a Medical Clearance Form and a Rules and Regulation Form, then hand it all to your dojo heads so it can be forwarded to the tournament organisers with your payments.
   
3. Dojo Heads: please complete the XL Spreadsheet, “Competition Nomination Form” listing all competitor details.
   Please note: This form of nomination ensures that competitors’ names and information is not misread so as to prevent competitors being placed in the wrong division.
   
4. Dojo Heads: Please list all judges and table officials attending, the tournament on the 2nd XL Spreadsheet.
   To be fair at the tournament we would like each dojo participating to provide judges and table officials, as the tournament can not be a success without their help.
